The Health Data Coalition (HDC) is a not-for-profit organization supporting primary care
providers across British Columbia to use data for quality improvement (QI). Through its flagship platform, HDC Discover, NPs can securely access anonymized EMR data to reflect on their own practice, identify trends, and engage in team-based QI initiatives. HDC is funded by the Family Practice Services Committee (FPSC) and works in partnership with the Nurse Practitioners of BC (NNPBC) to ensure NPs are fully supported in data-
informed practice and continuous improvement.

 

What HDC Offers NPs

  1. Data-Informed Practice Support

HDC works directly with NNPBC to make data meaningful and actionable for NPs. The HDC Discover platform helps you:

    • View your own panel’s population data (e.g., chronic disease, mental health, preventive care).
    • Compare anonymized trends with peers and provincial benchmarks.
    • Identify gaps in care and track progress over time.

2. Clinical Services Manager (CSM) Support

CSMs provide personalized guidance through:

    • HDC Discover enrolment and setup.
    • Orientation and onboarding sessions.
    • Ongoing data interpretation and QI project facilitation.
    • Panel management and workflow improvement strategies.
    • Community-level data discussions to inform Primary Care Network (PCN) and Division initiatives.

Learning, Tools, and QI Resources

HDC offers flexible learning pathways for NPs who want to enhance their QI capacity:

  • Webinars and video tutorials on using HDC Discover.
  • Step-by-step templates for disease-specific QI projects (e.g., COPD, diabetes, mental health).
  • Independent or PSP-facilitated QI cycles with or without a coach.
  • Community of practice opportunities to share learning across sites and disciplines.

 

All tools are aligned with provincial clinical guidelines and NNPBC’s QI framework, helping NPs turn data into meaningful action.

Compensation for QI Work

Publicly funded Primary Care NPs can claim compensation through NNPBC’s Quality Improvement Initiatives, which provide up to 40 hours of funded QI work per fiscal year at $122/hour.

Eligible activities include:

  • Enrolment in HDC Discover (up to 4 hours).
  • Participation in HDC- or PSP-supported QI projects.
  • Independent QI projects approved by the NP PIP team.
  • Completion of a data-informed QI cycle supported by HDC.

 

Review QI Policy. 

Submit hours using the QI Claim Form within 90 days of completing your work.

Provincial NP HDC Group

NNPBC and HDC co-host a Provincial NP group within HDC Discover.
This group brings together nurse practitioners across BC who are using HDC data to share learning, compare practice patterns, and advance NP-led QI.
Joining the group allows NPs to:

  • View collective NP metrics across the province.
  • Contribute to shaping NP-specific indicators and practice insights.
  • Network with peers who are integrating data into daily care.
