Job summary

Working independently and participating as a member of a multidisciplinary health care team, the Primary Care Nurse is responsible for direct patient/client care at
Elevate Medical Clinic in Campbell River, British Columbia
Delivers quality patient/client care in accordance with established standards, policies, procedures and the goals, objectives, guidelines and direction of the Vancouver Island Health
Authority by:
- Performing nursing skills, including delegated medical functions and crisis intervention, including emergency assessment and care, point of care testing, specimen acquisition, in accordance with established policies, protocols and procedures, and arranging referrals where necessary.
- Assessing, planning, implementing, documenting and evaluating the care needs and goals for individual patients/clients and their caregivers/families.
- Providing information to staff as required, including patient/client specific care instruction.
- Reviewing patient/client care needs and treatment goals. Planning, implementing and evaluating changes to the written Treatment Plan as required.
- Providing nursing assessment and consultation services for patients/clients at the Elevate Medical Clinic.
- Utilizing a variety of established health education, promotion and service interventions, such as the Well Child Program, and Pre/Post-natal Care;
- Optimizing capacities for health, by modeling, counseling and teaching life skills for individual patients/clients and their caregivers/families;
- Immunizing children and adults
Elevate Medical Clinic provides comprehensive primary health care services to Campbell River residents. Our team works together to meet all primary health care needs, including routine screenings, medication renewals, simple procedures, referrals to specialists, maternity care, and ongoing follow-up. Elevate Medical Clinic also offers wrap-around care for marginalized individuals, ensuring low-barrier access to primary health care for those facing mental health struggles, substance use issues, homelessness, and racism.
We work together to help reduce the systemic barriers people face when navigating the healthcare system. Our team provides culturally safe and trauma-informed care.

 

Key responsibilities

Apply nursing process as required and described in BC College of Nurses and Midwives Standards of Nursing Practice.
Demonstrate clinical competence.
Work effectively as part of a multidisciplinary team, with other staff, (NP's, Social Worker and Clinical Pharmacist) and with patients/clients and their caregivers/families.
Demonstrate conflict resolution, critical thinking and problem-solving skills.
Utilize sound judgment, good observation and assessment skills, tact and empathy.
Fact-find and seek out information and/or resources.
Intervene in crisis or difficult situations.
Organize and prioritize service needs and delivery of service.

Operate-related equipment, including computers.
Possesses the physical, emotional, and interpersonal capacity necessary to support marginalized and vulnerable patients with compassion, professionalism,
and respect, including those experiencing substance use disorders.

 

Qualifications

Remote Nursing Certification or eligible for Remote Nursing Certification to be obtained within one (1) year of hire.
Registration with BC College of Nurses and Midwives as a practicing RN(C) registrant.
Three (3) years' recent related experience; knowledge of available programs and services, their interrelationships, and their function in delivering care in the community; or an equivalent combination of education, training and experience.
Basic Life Support, Level C certificate. Advanced Cardiac Life Support (ACLS).

 

Compensation & benefits

Reports directly to manager at Vancouver Island Health
Under the Nurses' Bargaining Association (NBA) collective agreement with the BC Nurses' Union (BCNU), a Primary Care Nurse Level 3 (typically a Registered Nurse or Registered Psychiatric Nurse) earns an hourly base wage between $41.42 and $59.52, depending on experience and years of service.
This is a 0.6 FTE for Tuesday, Wednesday and Thursday 9:00 AM to 4:00 PM
