Job summary
Elevate Medical Clinic is seeking for a 1.0 FTE Nurse Practitioner to join its team. The clinic provides specialized service in providing wrap-around care to those that are marginalized and vulnerable and focus on low barrier access to primary care for those struggling with mental health, substance use issues, homelessness, and racism. The Nurse Practitioner will build their practice according to the Primary Care Network Nurse Practitioner Service Contract.
The clinic is a collaborative team of Nurse Practitioners and Family Physicians specializing in Opioid Agonist Therapy, including a Social Worker, Registered Nurse, and a Clinical Pharmacist. Many of the people who access care at our clinic face systemic barriers within the health care system and we work as a team to ensure we are reducing these barriers so people may access culturally safe and trauma-informed care. In applying for this position, you would bring compassion and strong desire to work with those who are vulnerable and marginalized.
Key responsibilities
* Provide comprehensive, patient-centred primary care across the lifespan, including assessment, diagnosis, treatment, health promotion, disease prevention, and chronic disease management.
* Conduct comprehensive health assessments, develop treatment plans, prescribe medications, order and interpret diagnostic tests, and provide ongoing monitoring and follow-up in accordance with British Columbia Nurse Practitioner scope of practice and standards.
* Deliver trauma-informed, culturally safe, and non-judgmental care to marginalized and vulnerable populations, including individuals experiencing substance use disorders, mental health challenges, homelessness, and complex social determinants of health.
* Collaborate within an interdisciplinary team consisting of Nurse Practitioners, an Opioid Agonist Therapy (OAT) physician, Registered Nurse, Clinical Pharmacist, and Social Worker to coordinate and optimize patient care.
* Provide patient education, counselling, and advocacy to support informed decision-making, self-management, and improved health outcomes.
* Develop therapeutic relationships with patients through compassionate, empathetic, and respectful communication.
* Maintain accurate and timely clinical documentation and ensure compliance with applicable professional, legal, and organizational standards.
* Participate in case conferences, care planning, quality improvement initiatives, and collaborative team meetings.
* Promote culturally responsive and inclusive care practices that recognize and address barriers to accessing healthcare.
* Liaise with community agencies, specialists, hospitals, and other healthcare providers to facilitate continuity and coordination of care.
* Practice in accordance with the standards, limits, and conditions established by the British Columbia College of Nurses and Midwives and applicable provincial legislation.
Qualifications
Current registration and licensure as a Nurse Practitioner with the British Columbia College of Nurses and Midwives
Experience in primary care, community health, addictions medicine, or mental health preferred
Strong communication, clinical judgment, and collaborative practice skills
Commitment to culturally safe, compassionate, and non-judgmental care
OAT training is an asset, but not required
Compensation & benefits
The successful candidate will agree to provide a minimum number of hours under the service contract which is approximately 1680 hours of service per year, distributed equitably throughout the year with no more than 90 hours in any biweekly period.
The remuneration under the PCN NP Service Contract is currently $171,533.00 annually with increases over the next 2 years
with an additional compensation overhead allowance of $75,000 per year to help cover operational/business costs
