Job summary
The Lookout Clinic (Surrey North Community Health Center) is a Fraser Health–funded clinic operated by Lookout Housing and Health Society. Since 1992, it has partnered closely with Fraser Health to deliver sustainable, wraparound public health programs alongside primary care for the North Surrey (Whalley) community.
Key responsibilities
Services include STI/HIV/HCV screening and treatment, wound care, addiction medicine, and longitudinal chronic disease management. Working within a trauma informed, team based model (NPs, FPs, RNs), the Nurse Practitioner provides primary care and addiction medicine support, including OUD care such as OAT, iOAT, and fentanyl patch prescribing.
Qualifications
• Licensed or eligible with BCCNM (NPs) or CPSBC (FPs)
• Recent clinical experience; experience in primary care and working with vulnerable populations is a strong asset
• CPR-HCP required
• Opioid Agonist Therapy training or willingness to obtain is an asset

Compensation: 1.0 FTE - $169,867 – $181,192 annually with no overhead
