Job summary

The Lookout Clinic (Surrey North Community Health Center) is a Fraser Health–funded primary care clinic operated by Lookout Housing and Health Society. Since 1992, it has partnered closely with Fraser Health to deliver sustainable, wraparound public health programs alongside primary care for the North Surrey (Whalley) community. Services include STI/HIV/HCV screening and treatment, wound care, addiction medicine, and longitudinal chronic disease management. The role offers meaningful, equity‑focused work with a reduced panel size tailored to the clinic’s population. 1.0 or 0.5 FTE

 

Key responsibilities

Working within a trauma‑informed, team‑based model (NPs, FPs, RNs), the Nurse Practitioner provides primary care and addiction medicine support, including OUD care such as OAT, iOAT, and fentanyl patch prescribing.

 

Qualifications

Licensed to practice as a Nurse Practitioner in British Columbia. Experience providing OUD care.

 

Compensation & benefits

1.0 FTE — $169,867 to $181,192 annually, with no overhead.
