Job summary
Sunshine Coast Health Centre (SCHC) and Georgia Strait Women’s Clinic (GSWC) provide residential treatment for addictions, mental health, trauma, PTSD, and OSI in gender-specific programs. We are licensed by Vancouver Coastal Health and accredited by Accreditation Canada.
We are seeking a Case Manager to join our multidisciplinary clinical team. This role is open to Registered Psychiatric Nurses (RPNs), Registered Occupational Therapists (OTs), and Registered Social Workers (RSWs) with experience in mental health and addictions.
Under the direction of the Nursing Director, Medical Director, and Psychiatrist, the Case Manager is responsible for comprehensive client case management, including assessment, treatment and care planning, implementation, evaluation, documentation, and discharge planning. The role works closely with clinical, medical, and operational teams, serving as a bridge among them and ensuring that each client’s care remains the focus. This position also works closely with external referral partners to ensure safe, ethical, and person-centred care.
Key responsibilities
Conduct mental health and psychosocial assessments, treatment planning, and crisis intervention
Coordinate multidisciplinary care plans and collaborate with physicians, psychiatrists, and therapists
Case managers assigned clients throughout admission, treatment, and discharge
Liaise with referral sources, hospitals, and community partners
Participate in case conferences, staff meetings, and quality improvement initiatives
Contribute to documentation, reporting, and discharge summaries
Support group facilitation and provide coverage as required
Qualifications
One of the following:
Registered Psychiatric Nurse (RPN) registered with BCCNM
Registered Occupational Therapist registered with COTBC
Registered Social Worker (RSW) registered with BCCSW
Diploma or degree from a recognized program in nursing, occupational therapy, or social work
Minimum 1 year of recent experience in mental health and/or addictions (or equivalent combination of education, training, and experience)
Knowledge of mental illness, substance use disorders, and evidence-based treatment approaches
Familiarity with CBT, DBT, and Motivational Interviewing
Strong written and verbal communication skills
Self-discipline to work independently
Passion to work collaboratively within a multidisciplinary team
Valid BC driver’s license and ability to travel locally as required
Flexibility to work weekdays, evenings, and weekends, including occasionally on-call or coverage for Nursing Supervisor and Nursing Director
