Susan Brown

Susan has been practicing as an Advance Practice Nursing Leader and Clinical Nurse Specialist for over 12 years across Vancouver Coastal Health (VCH) and Fraser Health (FH). She has extensive experience in community/home health specializing in the older adult population with chronic life limiting illnesses, complex care planning and risk assessment, and care transitions. Susan has led a number of quality improvement initiatives related to home health transformation and redesign of services to meet the needs of seniors and their families, as well as clinical design and development of the Fraser Health Seniors Clinic in Surrey B.C. Her current focus and interest is system redesign and creating a strategy for integrating palliative approaches to care in residential care.
