Medical Orders for Scope of Treatment (MOST)

Do you work with the MOST form, also known as the Goals of Care (GOC) form at BC Cancer and the Options for Care (OFC) form at Providence Health Care?

• Are you a prescriber of MOST/GOC/OFC?
• Do you interpret and follow MOST/GOC/OFC orders?
• Do you talk to people about their MOST/GOC/OFC designation?

If any of these describe you, the Ministry of Health would like to hear from you. Your input will inform a provincial MOST policy that is being developed. The survey takes approximately 10 minutes and your responses will be anonymous.
