Through its flagship platform, HDC Discover, NPs can securely access anonymized EMR data to reflect on their own practice, identify trends, and engage in team-based QI initiatives. HDC is funded by the Family Practice Services Committee (FPSC) and works in partnership with the Nurse Practitioners of BC (NNPBC) to ensure NPs are fully supported in data-<br \/>informed practice and continuous improvement.<\/p><p>\u00a0<\/p><p><span style=\"color: #467b87;\"><em><strong>What HDC Offers NPs<\/strong><\/em><\/span><\/p><ol><li><strong>Data-Informed Practice Support<\/strong><\/li><\/ol><p style=\"padding-left: 40px;\">HDC works directly with NNPBC to make data meaningful and actionable for NPs. The HDC Discover platform helps you:<\/p><ul><li style=\"list-style-type: none;\"><ul><li>View your own panel\u2019s population data (e.g., chronic disease, mental health, preventive care).<\/li><li Care NPs can claim compensation through NNPBC\u2019s <b>Quality Improvement Initiatives<\/b>, which provide up to <b>40 hours of funded QI work per fiscal year<\/b> at <b>$122\/hour<\/b>.<\/p><p class=\"p1\">Eligible activities include:<\/p><ul class=\"ul1\"><li class=\"li1\">Enrolment in HDC Discover (up to 4 hours).<\/li><li class=\"li1\">Participation in HDC- or PSP-supported QI projects.<\/li><li class=\"li1\">Independent QI projects approved by the NP PIP team.<\/li><li class=\"li1\">Completion of a data-informed QI cycle supported by HDC.<\/li><\/ul><p>\u00a0<\/p><p class=\"p1\"><a href=\"https:\/\/www.nnpbc.com\/np-content\/index.php\/quality-improvement-initiatives\/\" target=\"_blank\" rel=\"noopener\"><b>Review QI Policy.\u00a0<\/b><\/a><\/p><p class=\"p1\"><b>Submit hours using the <\/b><a href=\"https:\/\/www.nnpbc.com\/np-content\/index.php\/quality-improvement-initiatives\/\"><span class=\"s1\"><b>QI Claim Form<\/b><\/span><\/a><b>
